The Network is in the process of becoming a Managed Clinical Network (MCN) and is made up of an executive steering group which steers the work of the 2 sub group forums, the clinical & research forum and the workforce and education forum. The executive steering group is chaired by Dr Susan Picton and has representatives from a wide range of organisations including representatives from each hospice in the Region, a neonatal rep, sub group forum leads, a ccg rep, a research rep, a rep from the APPM (Association for Paediatric palliative medicine), community nursing rep and specialist nursing rep. The clinical forum includes representatives from each district general hospital and community nursing team in addition to allied health professionals, an education representative and a social care representative. The workforce and education forum is a multi agency group of people working to bring together best practice in training and education across the Region and Nationally.
Antenatal referral form
The antenatal care referral form has been developed for use across the four children's hospices in the Yorkshire and Humber region. It is for professionals involved in antenatal care where there is a diagnosis or recognition of a possible life-limiting condition of the baby. Please complete the form, with consent of the mother, and send to the email contact for the chosen hospice listed at the bottom of the form.
