Here we highlight useful resources available to professionals for use when caring for babies, children and young people with life-limiting and life-threatening conditions.

The antenatal care referral form has been developed for use across the four children's hospices in the Yorkshire and Humber region. It is for professionals involved in antenatal care where there is a diagnosis or recognition of a possible life-limiting condition of the baby. Please complete the form, with consent of the mother, and send to the email contact for the chosen hospice listed at the bottom of the form.
This leaflet helps professionals to recognise babies, children and young people who may benefit from palliative services and care. It also aims to highlight some key areas to consider at various stages of their care.

This document aims to support health care professionals in planning and coordinating the urgent transfer of children and young people from hospital to home or hospice in the last hours or days of life. The Child and Young Person's Advance Care Plan (CYPACP) is a national website with resources available to help professionals to understand and complete Advance Care Plans. Resources are available for patients, families and professionals.

The Association for Paediatric Palliative Medicine (APPM) represents all doctors, nurses and allied health professionals working in the area of paediatric palliative medicine in hospital, hospice and community settings. It seeks to support practice and training for all doctors and other health professionals working in this area of medicine.
The APPM Formulary is an important core resource for the prescriber working in paediatric palliative medicine. It brings together all available paediatric palliative prescribing information in a single volume, utilising up-to-date published research and consensus expert opinion. It provides new guidance on existing medication and introduces newer drugs pertinent to the field of paediatric palliative medicine. Get In Touch
